Membership Application
You may e-mail a request for general information. To apply for entrance, follow the instructions below:
- Download the membership application form as a .pdf then, print, complete the application form and post mail it to us along with the requested documentation (to read this file must have a copy of the (Adobe Acrobat Reader installed on your computer - it's free from Adobe) OR,
- Print this page and form from your web browser, complete the application form and post mail it to us along with the requested documentation.

Application Fee: $150.00
This fee is for processing all documents, a framed PAg certificate, & official stamp (seal).
Transfer Fee: $75.00
This fee is for processing transferring members from other
institutes. Please complete the application form (found above)
with current information and send to BCIA with a current
resume and the required payment. Contact previous institute for
transfer of file to BCIA.
Dual Fee: $75.00
This fee is for processing members who would like dual
membership with BCIA. Please complete the application form
(found above) with current information and send to BCIA with a
current resume and the required payment. Contact present
institute for copy of file to be sent to BCIA. An invoice will then
be sent to member for balance of dues for the year.

Please enclose official transcripts for degrees & other credits.
